Contract Navigators for Northland

About ENCompass Navigation Services.

ENCompass provides a Navigation Service for people and their families/whanau.  We are seeking to engage independent Contractors throughout Northland who are interested in working on a fee for service basis. 

About the Role:
It's about outcomes.  You will be matched to work with individual people and their families/whanau to meet defined goals and outcomes within a specified timeframe, which may vary between 8 and 24 weeks.  Over an average of 4 – 6 hours per week (based on need), you will provide practical guidance and support, collaborating with people and families/whanau to:

plan a pathway to achieve agreed goals
effectively engage with support services
communicate, mediate, and resolve issues and concerns with service providers / agencies
respond to barriers by finding solutions
prioritise and plan for the future
identify and strengthen natural supports and links to local communities

Skills and Requirements

You will have:

A relevant tertiary level qualification in health or human services – e.g. nursing, social work, special education, or allied health roles such as occupational therapy, speech language therapy, physiotherapy, etc
A person-centered, holistic, and collaborative approach to working with people in a health and social service setting
Case management experience including assessment, planning, negotiation, coordination, setting objectives, defining strategies, monitoring implementation, and reporting on outcomes
Strong communication skills, including negotiation and mediation
Creative thinking and problem-solving abilities
Knowledge of community agencies and resources and strong local community networks
Able to use client management systems to report and record activities
A driver's licence, reliable vehicle, computer and internet connection
Demonstrated experience in the application of the principles of the Treaty of Waitangi in a Social Services setting
A satisfactory Ministry of Justice clearance

Services Coordinator

Bring your previous case management experience to coordinate both the NorthAble Very High Needs Service (VHN) and ENCompass Navigation Service.   This full time position is based in Whangarei and will operate across Northland. 
These roles require a good understanding of health, disability, education and other social services in Northland, with proven experience in developing and maintaining close working relationships with all stakeholders.  An ability to develop and work within new programmes and systems is also required. 
NorthAble VHN Service Coordinator:
The NorthAble VHN Service Coordinator works closely with individuals and families. Having experience in assessment and coordination of services for people with disability, and the ability to follow up on leads, is an advantage.
Based in Whangarei, there will be some travel involved to meet with individuals/family/whanau living throughout Northland.  Company pool vehicles are accessed via an internal bookings system.  Working after-hours may be required occasionally, for which time in lieu is provided. 
Promotion of the NorthAble VHN Service, planning and documenting activities with individuals and their family/whanau, signing off on expenditure, regular budget reviews and preparing reports to the funder are key performance indicators of the position. 
The VHN Coordinator will report directly to the Client Services Manager.
ENCompass Navigation Service Coordinator:
The ENCompass Navigation Coordinator (NavCo) position will:
establish and maintain positive stakeholder relationships
provide ongoing training, mentoring and support to contracted Navigators
be a point of contact for referring agencies, receiving referrals, and matching these to the most suitable Navigator, overseeing and monitoring of each engagement including ensuring outcomes are achieved and reporting processes met within agreed time frames
prepare qualitative and quantitative reports as required
The NavCo role receive supports, training and direction from the ENCompass Navigation Team.

Previous case management experience in a disability/health/education/other social services setting
Ability to write plans that are outcome driven and goal orientated
Advanced administration and computer skills, including MS Office applications and database management
Contract and financial reporting experience
Excellent written and verbal communication skills
Demonstrated successful time management
Understanding of Quality processes
Able to work with people with disabilities
Uses initiative and is motivated
Clean, full drivers licence (ability to drive a manual vehicle)
Knowledge / training in Maori Cultural Safety and Practice
Knowledge of community agencies and resources
Available to work afterhours on occasions, if required
Ability to obtain a satisfactory Ministry of Justice clearance

Relevant tertiary qualification (Health, Social Services, Education etc)
Knowledge and understanding of the Northland disability sector
Experience in leadership and training of others in a formal learning environment
