NASC - Needs Assessment and Service Coordination

Information and support options to assist you to meet goals, NASC is a free service provided by NorthAble for people living with a disability.

If you meet the Ministry of Health’s (MoH) eligibility criteria, NASC will meet with you to explore your support options and goals. We can then discuss the information and support options that may assist you to meet your goals. These may include:

  • Manage household tasks
  • Self-care
  • Supported assistance in the community
  • Attendance at MOH and MSD funded activity programmes
  • Carer Support Subsidy providing time out for unpaid full time carers
  • Rehabilitation support
  • Residential Support Services
  • Support to live in your own residence

Referrals

Referrals can be made by the person with a disability, family/whanau, General Practitioner, or even your Health Professional Community Agency.

Assessment

When a Referral is accepted an initial assessment will be carried out by an Assessment Facilitator at a time and place of your choice. You may choose to have support people present. This assessment will firstly determine eligibility, and if accepted, the outcomes you wish to achieve. We will endeavor to meet your cultural needs.

The assessment is directed by the person and the assessment ends when the person is in agreement that all needs have been discussed and assessed fully.

The final report will be written by the Assessment Facilitator and a copy will be made available to the person.

Service Facilitation

The Service Coordinator works with the person to identify possible support options to help reach goals and positive outcomes. Once these options have been identified and agreed on, the Service Coordinator will make referrals to appropriate services. Once referrals have been made the Service Coordinator will continue to monitor progress.

Our NASC Maori Liaison is also available to provide culturally appropriate support.
